Medication on Campus
If your child needs to take medication during the school day, please complete the required forms located in the Clinic Documents section of our Forms & Documents page.
A separate medication authorization form is required for each medication and must be submitted to the school clinic along with the medication.
Medication Guidelines
- All medications must be delivered to the school clinic by a parent or legal guardian.
- Students may not carry medication on campus unless they have written authorization for a medically necessary condition, such as asthma, diabetes, seizures, or severe allergies.
- Students who are approved to carry medication must have a completed Permission to Carry Medication form on file in the clinic.
- Any student found carrying medication without the required authorization will have the medication confiscated and may be referred to school administration.

Field Trip Medication
Students may not carry medications on field trips, including prescription medications or over-the-counter medications (such as Tylenol), unless they have an approved Permission to Carry Medication form on file.
Before the Field Trip
- A parent or legal guardian must deliver all medications to the designated field trip supervisor.
- Medications must be in their original prescription bottle or original manufacturer’s packaging.
If a Student Brings Medication Without Authorization
- The medication will be confiscated.
- Parents or guardians will be contacted.
- The student may be required to leave the field trip at the parent’s or guardian’s expense.
Health Conditions & Care Plans
If your child has a medical condition that requires special care or accommodations during the school day, please contact the school clinic for the appropriate forms.
Some forms require only a parent or guardian’s signature, while others must be completed and signed by your child’s licensed healthcare provider.
